1711.A.&B.&D.: Based on observations at 11:30 a.m., child to staff ratio failed to be met for the following group of children: 10 children, ages 2-months-to-1-year-old, with 1 staff, S3. The required ratio for this group of children is 5 children per 1 staff person. This was corrected prior to the specialist leaving the center. Corrective Action: Effective 12/8/2025, S1 stated she will retrain staff on child to staff ratio requirements and verifying there is enough coverage before taking breaks, to ensure compliance with this regulation.
