Based on observations at 12:50 p.m., S2 and S3 failed to ensure children were being supervised. The Specialist observed S2 and S3 using their phones during nap time while supervising 12 children, ages 2-to-10-years-old. Corrective Action: Effective 6/10/2026, S1 stated she will have all staff put their phones in the kitchen upon arrival to the center to ensure compliance with this regulation.

Based on observations at 12:55 p.m., the outdoor area was not free of hazards. The Specialist observed an unsecured pvc pipe border, around the play area, disconnected in several places. This was not corrected. Corrective Action: Effective 6/10/2026, S1 stated she will attach and secure the pieces of pvc pipe as soon as possible to ensure compliance with this regulation.

Based on record review at 10:30 a.m., S1 lacked documentation that 1 of 4 staff, S4, received orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S4 lacked documentation of a completed DCFS Mandated Reporter training and was providing sole responsibility of children. This could not be corrected during the inspection.
