1507.B Based on record review on 4/21/2026 at 12:56 p.m., S1 failed to maintain documentation of a daily attendance record for Staff and Owners, to include the time of arrival and departure for February 2026, March 2026, and April 2026. Corrective Action: Effective 4/30/2026, S1 stated she will keep attendance records in a secure place to ensure compliance with this regulation.

1507.E. Based on record review on 4/21/2026 at 10:01 a.m., S1 failed to maintain documentation of a daily attendance record for visitors to include the departure times, and purpose of the visit for O1, O2, O3, O4, and O5 for the following days: 4/15/2026, 4/16/2026, and 4/20/2026. Corrective Action: Effective 4/30/2026, S1 stated she will check the visitor's log for proper documentation after each visitor to ensure compliance with this regulation.

Based on record review and interviews 4/21/2026 at 1:30 p.m., the center failed to have a Behavior Management Policy that clearly states ALL types of positive discipline that are used and all methods of discipline that are prohibited. On an unknown date, S1 used a prohibited method of discipline on the children in the center S1 stated that when she is frustrated and overwhelmed, she has yelled at the children in the center. 1707.A.1.&2 Based on record review on 4/21/2026 at 12:25 p.m., there failed to be documentation of a director or director designee who is an on-site, full time staff person at the center during the daytime hours of operation responsible for planning, managing, and controlling the center's daily activities, as well as responding to parental concerns and ensuring that minimum licensing requirements are met. Based on observations and interviews on 4/21/2026 at 12:30 p.m., S1 failed to devote time to supervising the children, meeting the children's needs, and participating with the children in activities. While S1 was in the office, C11, age 4-years-old, was placed in a time-out in a seat in the hallway for four minutes with her back turned. No other staff were in the hallway with C11 during this time before S1 moved C11 to a classroom. Based on record review on 4/21/2026 at 12:45 p.m., S1 failed to have documentation that S4 (first day present at the center 4/20/2026) completed, LDE Key Training Module 1, 2, and 3, the DCFS online Mandated Reporter Training before assuming sole responsibility for any children. The Specialist observed S4 with sole responsibility for 3 infants. Corrective Action: Effective 4/30/2026, S1 stated she will have all staff complete the required training on their first day to ensure compliance with this regulation.

1723.F. Based on record review on 4/21/2026 at 11:30 a.m., S1 failed to ensure that S4 (DOH 4/20/2026) had a current certification in pediatric first aid and CPR within 90 calendars from the date of hire and prior to assuming sole responsibility for the children. The Specialist observed S4 supervising 3 infants, alone in the classroom. This was corrected at 12:00 p.m. when S2 went into the classroom. Corrective Action: Effective 4/30/2026, S1 stated she will have all new staff paired with a staff member with all current trainings to ensure compliance with this regulation.

1901.S.3. Based on observation and interview on 4/21/2026 at 11:15 a.m., S1 failed to properly dispose of soiled diapers in a closed container. Specialist observed soiled disposable diapers placed in the garbage can in the restroom with no lid on the garbage can. Corrective Action: Effective 4/30/2026, S1 stated she will have all trash cans replaced with lids to keep them secure and ensure compliance with this regulation.

1903.C. Based on observation and interview on 4/21/2026 at 10:45 a.m., S1 failed to have the outdoor area free of hazards. The Specialist observed a long wooden board leaning against the gate of play yard 1 between both play yards. The board contains screwed-in coat hooks with the screw exposed at the back of the board. S1 moved the board off the play yard at 12:05 p.m. Corrective Action: Effective 4/30/2026, S1 stated she would check the yard before bringing the children outside to ensure compliance with this regulation.

1903.E.6. Based on observations on 4/21/2026 at 11:00 a.m., S1 failed to ensure crawlspaces and mechanical, electrical, or other hazardous equipment is not inaccessible to children. The Specialist observed an A/C unit on play yard 2 accessible to children as the lattice enclosing the unit was broken on the front and left side, resulting in the lattice leaning open. Corrective Action: Effective 4/30/2026, S1 stated she will have the A/C unit lattice properly enclosed and checked before allowing children on the play yard to ensure compliance with this regulation.
