No other staff were in the hallway with C11 during this time before S1 moved C11 to a classroom. Specialist observed soiled disposable diapers placed in the garbage can in the restroom with no lid on the garbage can. Corrective Action: Effective 4/30/2026, S1 stated she will have all trash cans replaced with lids to keep them secure and ensure compliance with this regulation.

The Specialist observed a long wooden board leaning against the gate of play yard 1 between both play yards. The board contains screwed-in coat hooks with the screw exposed at the back of the board. S1 moved the board off the play yard at 12:05 p.m. Corrective Action: Effective 4/30/2026, S1 stated she would check the yard before bringing the children outside to ensure compliance with this regulation.

The Specialist observed an A/C unit on play yard 2 accessible to children as the lattice enclosing the unit was broken on the front and left side, resulting in the lattice leaning open.
