      1507.A.1. Based on record review at 2:30 p.m. on 4/17/2026, S1 failed to ensure the daily attendance log for children included the name of the person to whom the children were released on 4/14/26 for 23 of 36 children. Corrective Action: Effective 04/29/2026, S1 stated she will remind all parents to sign out the children before leaving the center and the teachers will also review Brightwheel to ensure compliance with this regulation.

      1507.B.1.2.3. Based on record review and observations at 1:45 p.m. on 4/17/2026, there failed to be a daily attendance record for all staff members for the weeks of 04/09/2026 - 04/10/2026, and 04/13/2026 - 04/17/2026. Although the Specialist observed S1 on the premises, there was no paper or electronic documentation available to accurately reflect S1's presence at the child care facility at any given time. Corrective Action: Effective 04/29/2026, S1 stated she will sign in and out when present at the center to ensure compliance with this regulation.

      1509.A.8.b.i. Based on interview and record review on 04/17/2026 at 12:42 p.m., although the center has a behavior management policy implemented, S7 (date of hire 05/01/2025), used a prohibited method of physical punishment on 04/14/2026 at 2:23 p.m., as she placed C1, age 1-year-old, under hot water during a diaper change, causing 2nd degree burns. S1 stated S7 was terminated on 04/15/2026. Corrective Action: Effective 04/29/2026, stated she will retrained staff in behavior management policy to ensure compliance with this regulation.

      1711.A.D.1. Based on observations at 2:16 p.m., S6 failed to meet the required child to staff ratio for infants. The Specialist observed S6 caring for 7 children, ages 6-months to 2-year-old. The required ratio for infants is 5 children per 1 staff person. One additional staff was needed to meet ratio. Ratio was met at 2:18 p.m. when S2 rearranged the children by age and went into a classroom. Corrective Action: Effective 4/29/2026, S1 will ensure sufficient staff is present at all times to ensure compliance with this regulation.

    • CCCBC-Based Determinations of Eligibility for Child Care Purposes Required for Volunteers and Staff --Not1807.B

      1807.B. Based on record review at 11:00 a.m. on 4/17/2026, S1 failed to have documentation of eligible staff CCCBC determination available for S8, DOH 12/24/25, on the center’s CCCBC roster at all times. This was corrected prior the Specialist departure. Corrective Action: Effective 04/29/2026, S1 stated she will check the CCCBC roster monthly to ensure all staff have been added to the center’s roster to ensure compliance with the regulation.
