      Based on observation and record review, the licensee did not comply with the section cited above in not testing for lead water before the due date of January 1st 2023 which posed a potential health, safety or personal rights risk to persons in care. Facility tested for lead on 10/20/2023. POC Due Date: 06/20/2024

Written POC shall also describe how the Licensee shall prevent future recurrences of this deficiency. (During the inspection, the owner accessed Guardian and associated all four staff to her facility. This deficiency is considered corrected and cleared as of today.) 8 9 10 11 12 13 14 This evidence is not met, as evidenced by records review, Licensee failed to ensure all employees had a criminal record clearance associated to the facility prior to employment, posing an immediate health and safety risk to children in care.
