      Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] LS was reviewing the employee files and did not find the documents needed for background screening for an employee. LS reviewed the social security number in Clearinghouse and did not find the employee being screened. Provider stated the employee is under the age of 18 and that is why she was not screened. LS provided technical assistance from the handbook: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B.       Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B General Requirements [SR] LS observed the child care facility had a child discipline policy but it was not followed. Personnel needs to redirect children to another activity and follow child care discipline policy. LS provided technical assistance from the handbook: 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies. CCF Handbook, Section 6.3, B & C and 2.5.3, E Health Requirements [SR] LS interviewed teachers whom stated an incident occurred regarding a 4 year old child falling and hitting her head on the handwashing sink. Director nor the teachers reported the incident to the child's family nor wrote up a report. LS provided technical assistance from the handbook: 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.       Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Enforcement [SR] LS interviewed Director and teachers in which both stated they have witnessed an incident as a suspected child abuse and neglect. Director stated a child was inappropriate in her behavior in which she would try to touch other children's private parts as well as adults. LS asked Director if she reported suspected child abuse or neglect from her behavior and Director stated no. LS interviewed another teacher who stated the office admin had been aggressive to several children and to the point one got hurt in the past. The teacher had told the Director and a report was never reported to the hotline. Director did not file this report in the employee's files. LS provided technical assistance from the handbook: 8.2 Child Safety Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. CCF Handbook, Section 4.2.2 and 4.5, C (Section 2.1 Health and Safety, Page 3) Refer to the supplemental for the staff member who did not complete the required training within the time frame. 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs.
