C.: Based on record review at 1 p.m., S1 failed to have documentation that 1 of 7 staff, S5, received orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S4, DOH: 5/19/2025, completed LDE Key Training Module 1 on 6/4/2025 and DCFS Mandatory Reporter Training on 6/9/2025. Corrective Action: Effective 8/4/2025, S1 will have new staff to complete required trainings prior to starting work to ensure compliance with this regulation.
