The Specialist reviewed video footage from 3/12/2026, and observed S16 lift C2 by the one arm between her wrist and elbow from her nap mat to put her in time-out. Corrective Action: Effective 3/30/2026, S1 will review proper lifting at the center's next staff meeting on 4/6/2026 to ensure compliance with this regulation.

Once inside, the individual entered an occupied classroom. The police officers followed the individual into the center and escorted him off the premises.
