Based on record review at 10:30 a.m. on 5/11/2026, S2 reviewed center's video and discovered S14 failed to adhere to proper lifting techniques when she lifted C3, 2-years-old, by the left arm three times then throwing him towards the wall, picked up C3 by both arms then slams him on the changing table and picked C3 up by both arms and put him on the floor. Corrective Action: Effective 5/19/2026, S1 will retrain staff on proper lifting techniques to ensure compliance with this regulation.

C.: Based on interviews at 12:00 p.m., S1 failed to notify the department and DCFS Child Welfare within 24 hours of the following critical incident: On 12/1/2025, time unknown, O1 alleged an unknown staff member attempted to feed C1 a bottle which was too hot and C1 failed to have a diaper change. O1 alleged this occured on 11/25/2025. Corrective Action: Effective 2/4/2026, S1 will review the Critical Incident policy with all staff to include the reporting requirements to ensure compliance with this regulation.
