1509.A.8.a.&b.: Based on record review on 5/11/2026, at 10:30 a.m., although the center developed and implemented a written behavior management policy, C3 and C4, both 2-years-old, was subjected to prohibited methods of discipline on 5/7/2026 by S14 (DOH 3/23/2026). Based on observations on 5/19/2026, at 9:45 a.m., children failed to be under supervision at all times. During the walk through, Specialist observed 3 children, ages 6-year-old-to-8-year-old, in a classroom alone. The issue was corrected prior to the Specialist exiting the classroom. Based on record review at 10:30 a.m. on 5/11/2026, S2 reviewed center's video and discovered S14 failed to adhere to proper lifting techniques when she lifted C3, 2-years-old, by the left arm three times then throwing him towards the wall, picked up C3 by both arms then slams him on the changing table and picked C3 up by both arms and put him on the floor. Corrective Action: Effective 5/19/2026, S1 will retrain staff on proper lifting techniques to ensure compliance with this regulation.

C.: Based on interviews at 12:00 p.m., S1 failed to notify the department and DCFS Child Welfare within 24 hours of the following critical incident: On 12/1/2025, time unknown, O1 alleged an unknown staff member attempted to feed C1 a bottle which was too hot and C1 failed to have a diaper change. O1 alleged this occured on 11/25/2025. Corrective Action: Effective 2/4/2026, S1 will review the Critical Incident policy with all staff to include the reporting requirements to ensure compliance with this regulation.

Based on record review/observations at 9 a.m., S9 (DOH: 1/26/2026) failed to have current certification in pediatric first aid and CPR prior to assuming sole responsibility for 8 children, ages two-year-olds. This was corrected during the inspection. Corrective Action: Effective 2/4/2026, S1 will require new staff members to have current certification in pediatric first aid and CPR training approved by the department to ensure compliance with this regulation.

1719.A Based on record review at 12 p.m., S9 (DOH: 1/28/2025) failed to receive orientation which included center-specific orientation to the policies and practices of the center within seven days of the first day present at the center.

1807.B. Based on record review at 10:36 a.m., a CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for S4 (DOH:1/6/2025) prior to being present at the center and performing services. According to center records, S4 worked 1/6/2025 - 1/10/2025, 1/13/2025 1/17/2025, and 1/20/2025 without a CCCBC-based determination of eligibility. S4 received a CCCBC-based determination of eligibility on 1/24/2025.
