Based on observation/record review/interview on 3/11/2026, at 11:00 a.m., S1 failed to ensure the children daily attendance records on 3/6/2026, accurately reflected the children on the premises. A parent, signed their child, C1, 2-years-old, out in Procare at 12:39 p.m., but C1 was left in an unoccupied classroom at the center by the parent. At 12:56 p.m., C1 was found alone, outside on the playground by S3. S3 brought C1 inside the center and signed her back in at 12:39 p.m., due to her not leaving the premises. Based on record review/interview on 3/11/2026, at 11:00 a.m., S1 failed to have a qualified Director who is an on-site, full time staff person at the center during the day time hours of operation (prior to 9:00 p.m.) and responsible for planning, managing, and controlling the center's daily activities, as well as responding to parental concerns and ensuring that minimum licensing requirements are met. On 3/6/2026, from 11:30 a.m. - 5:00 p.m., 2 staff were present with 33 children. ¨ S3's classroom had 5 children ages: 6-months - 2-years-old. ¨ S4's classroom had 28 children ages: 2 - 11-years-old. The parent knowingly left C1 in the unoccupied classroom.
