Based on record review and interviews at 11:00 a.m., S1 failed to immediately notify O1 and the Department and DCFS within 24 hours of the following critical incident: On 07/21/2025, at 4:53 p.m., O2 used a prohibited method of behavior management when she grabbed C1, 1-years-old, by his right arm and yanked him downward towards the floor. O2 is not allowed to return to the center. Corrective Action: Effective 08/05/2025, S1 stated she will report all critical incidents and notifications to the Department and other entities as outlined in Bulletin 137 to ensure compliance…
