Based on record review on 5/26/2026, at 11:00 a.m., S1 failed to maintain documentation of a daily attendance record for all visitors, a Child Welfare Worker, and a NOPD Officer, to include arrival time, purpose of the visit and person accompanying visitor on 5/22/2026. Corrective Action: Effective 5/26/2026, S1 stated she will check the daily attendance daily to ensure visitors are signing in and out to ensure compliance with this regulation.

1515. C. Based on record review on 5/27/2026, at 10:00 a.m., S2 failed to maintain the confidentiality of C1, age 2- years- old, who was involved in an incident on 5/8/2026. S2 knowingly disclosed information concerning C1 and C7, to another parent. S2 disclosed information pertaining to C1’s developmental delays and that C7 continues to urinate on herself and isn’t potty trained properly. Corrective Action: Effective 5/26/2026, S1 stated she will retrain all of her staff on the centers confidentiality policy on 6/15/2026 to ensure compliance with this regulation.

1901.G. Based on observation at 11:00 a.m., S1 failed to ensure all of the centers equipment used by children was maintained in a clean and safe condition and in good repair. The Specialists observed a broken, metal picnic tables seat lying on the ground.

1903.E.5. Based on observation at 11:00 a.m., S1 failed to ensure the outdoor play space was enclosed with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The specialists observed the play yards fence unlocked and partially open with no working locking mechanism.

1919.A.B. Based on record review at 1:28 p.m., S1 failed to ensure the current weekly menu listing specific food items served for each day of the week was prominently posted, written or electronically, by the first day of each week and remain posted throughout the week.

1921.C. Based on observation at 1:30 p.m., S1 failed to have a completed evacuation pack to include the following: a list of area emergency phone numbers, a list of emergency contact information and emergency medical authorization for all enrolled children, an emergency pick up form, first aid supplies, hand sanitizer, wet wipes, and tissue, diapers for children who are not toilet trained and plastic bags for diapers, a battery powered flashlight and radio and batteries, food for children under the age of 4, including infant food and formula, disposable cups and bottled water.

C. Based on interviews at 10:30 a.m., S1 failed to immediately notify the Parent, and LDOE and DCFS within 24 hours of the following critical incidents: -In July 2024, S5 aggressively grabbed C2, age 3-years-old, by his upper body, when it appeared the child was putting an object in his ear. O2 was not notified. -In August 2024, O1, C1s mother, informed S1, that C1, age 3-years-old, told her S5 hit him.

Based on record review at 12:00 p.m., S1 failed to ensure the center's visitor's daily attendance record accurately reflected when a visitor was on the child care premises. -On 10/7/2024, 2 visitors failed to sign out.-On 10/21/2024, 2 visitors failed to sign out.-On 10/28/2024, 1 visitor failed to sign out.-On 10/29/2024, 3 visitors failed to sign out.

In July 2024, S5 aggressively grabbed a C2, age 3-years-old, by his upper body when it appeared the child was putting an object in his ear. S5 received a written warning due to her handling of C2, according to S2.

1727.A.B. Based on record review at 12:15 p.m., S1 failed to have documentation that S1 (DOH 2/19/24) and S5 (DOH 6/1/14) completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS.

1901.G.H. Based on observation at 9:40 a.m., S1 failed to ensure all of the centers equipment used by children was maintained in a clean and safe condition and in good repair. The specialist observed a broken, metal picnic table seat lying on the ground.
