Based on record review/interview at 12 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for each staff member, S4, prior to the person being present at the center or performing services. S4 started working at the center on 11/03/2025. S1 stated she was unaware S4 did not have an eligible CCCBC. Corrective Action: Effective 11/06/2025, S1 stated she will ensure new staff complete their CCCBC prior to working at the center to ensure compliance with this regulation.
