      Based on interview and record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that the volunteer at the facility did not have any immunizations on file at the center. LPA discussed with facility representative the importance of ensuring that all staff members have a copy of their required immunizations on file at the center before being allowed to work with children, for their health and safety. Missing immunizations are Measles, Pertussis, proof of TB testing, and influenza. POC Due Date: 10/13/2023

      Based on observation, interview, and record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. Per facility representative, the facility has not been tested for lead. Facility representative stated that the children do not drink from the drinking fountains and only drink bottled water. LPA reviewed the regulation for Lead Testing in centers opened before 2010. POC Due Date: 09/29/2023
