Based on observation at 1:45 p.m., S4 failed to devote her time to supervising the children, meeting the needs of the children, and in participation with the children in their activities. The Specialist observed S4 on her phone instead of supervising the children during naptime. Corrective Action: Effective 3/25/2026, she will sign a policy for cell phone and cell phones will no longer be allowed in the classrooms while the children are asleep to ensure compliance with this regulation.

Based on record review and interview at 2:15 p.m., S1 failed to have documentation that the entire center and play yard are checked after the last child departs to ensure that no child is left unattended at the center for the weeks of 3/2/2026 - 3/6/2026, 3/9/2026 - 3/13/2026, 3/16/2026 - 3/20/2026, 3/23/2026, and 3/24/2026. This could not be corrected. Corrective Action: Effective 3/25/2026, S1 stated she will print the necessary form, talk with staff, and make sure the visual checks are being completed daily to ensure compliance with this regulation.

Based on record review at 9:15 a.m., S1 failed to ensure the center's staff and owner's daily attendance record accurately reflected persons on the child care premises at any given time. S8 was signed in on Brightwheel as being present at the center but was not on the premises. S1 stated S8 has been working at another location for about a month, but it still clocks her in at this location. This was not corrected prior to the Specialist departure. Based on record review at 11:00 a.m., S1 failed to have documentation on file for O1 and O2 that included person's name, address, phone number, list of duties performed while at the center. Based on record review at 12:00 p.m., S1 exceeded the manufacturer's recommended capacity in the vehicle. On 7/10/2025, 14 children and 3 adults were transported in a 15 passenger vehicle. On 6/18/2025, 17 children and 6 adults were transported in a 15 passenger vehicle. The Specialist had previously observed the field trip documents for 6/18/2025, when the Specialist returned the documents had been changed to make it appear that the van was not over capacity. Based on observation/record review at 10:00 a.m., S1 failed to ensure all staff completed training in pediatric first aid and CPR within 90 calendar days from their date of hire. S5 (DOH: 03/05/2025) failed to have this certification. S1 had until 06/03/2025, to ensure they were trained. This was not corrected prior to the Specialist departure. Corrective Action: Effective 6/4/2025, S1 stated she will schedule S5's CPR and First Aid training by 6/5/2025 to ensure compliance with this regulation.

Based on record review/interview at 10:00 a.m., S3 and S5 failed to have documentation that included the liquid intake, food intake, disposition, bowel movements and eating and sleeping patterns of C1 (seven-month-old), C2 (seven-month-old), C3 (three-month-old). Corrective Action: Effective 6/4/2025, S1 stated she will complete a walkthrough of the center and make sure the daily infant reports are completed daily to ensure compliance with this regulation.

Based on record review at 12:15 a.m., S1 failed to have documentation that S2 and S7 received center-specific orientation to the policies and practices of the center and completed DCFS online mandated reporter training within 7 days of the first day present at the center.
