      Based on record review on 03/10/2025 at 11:00 a.m., S1, S2, and S3 failed to immediately notify LDOE of the following Critical Incidents: On 10/23/2024, S2 was notified by center staff of inappropriate touching between two children C1 and C4 (4- years-old); And on 02/26/2025, DCFS visited the center to inform them of a complaint of inappropriate touching between three children, C1, C2 and C3 (4-years-old). S1 stated they did not notify the Department, because there were no findings in fault of the center, and she did not believe the allegations to be true. The Specialist was unable…

      1507.E. Based on record review and interviews on 03/10/2025, at 11:15 a.m., S1 failed to ensure the daily attendance record for all visitor's logs included the name, date of visit arrival and departure times, and purpose of the visit. S2 stated they received a visit from DCFS on 02/26/2025, inquiring about an allegation of inappropriate touching involving three children. The Specialist observed visitor's logs for 2/26/2025 and there was no evidence that the DCFS worker had visited the center.
