The facility did not have documentation of a fire extinguisher being properly maintained to include being serviced and retagged timely, and/or with a current certificate. Child care personnel did not complete preservice training requirements prior to unsupervised contact with or care of children or, if supervised, within 90 days of beginning employment or service with the provider. (Section 3.1 and 3.2 Training Requirements, Page 7) [SR] The CCP listed on the Supplemental Sheet started working on 9/3/24 (over 90 days) & was supervising children with no other CCP in the classroom, and she has not completed the School Readiness OEL Pre-service training. Technical support was provided. 18 Training Requirements 18.1 Pre-service Timeframe All pre-service training requirements listed below must be completed by all program personnel, volunteers and substitutes, each as defined in this handbook, within 90 days of initial employment with any provider participating in the school readiness program. This timeframe does not start over if personnel change employment to another school readiness provider within this 90 days. Personnel who have not completed all pre-service training requirements may not be allowed any unsupervised contact with or care of children in a school readiness program.

Documentation of an accident or incident was insufficient in that it did not include the [signature of a parent/guardian]. CCF Handbook, Section 6.4, C Health Requirements [SR] An accident report written on 4/10/24 did not have a parent/guardian signature for the child listed on the Supplemental sheet. The parent signed it during this inspection. Technical support was provided. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.

The facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [the information was not shared with the parents in April]. CCF Handbook, Section 7.3, C.5. Technical support was provided. 5. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 (Section 19.2, number 2) Record Keeping [SR] FSC reviewed all 26 preschool children's records. The child listed on the Supplemental Sheet has an expired DH680/Immunization form. It expired on 5/18/23. Technical support was provided. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program.

The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C The staff listed on the Supplemental Sheet started working in the child care industry on 12/21/21 and has not completed the 5-hour Literacy training. Technical support provided. 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years.
