An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] There is a rotten board on the back porch with a portion of the board missing (with a mat on top of it) in the area leading to the playground. Director stated that children will be re-routed around that area or use another entrance to the playground until the board is replaced. Technical support provided. 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children.

The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ the use of fire extinguishers]. CCF Handbook, Section 4.2.3 and 4.2.5 The staff listed on the Supplemental Sheet started working on 8/11/23 and has not had the Fire Extinguisher training. Technical support provided. 4.2.5 Fire Extinguisher Training All child care personnel shall be trained in the use and operation of a fire extinguisher, at each facility they are employed, within 30 days of date of hire. The facility must maintain documentation that all child care personnel have completed training.

The Student Health Examination or equivalent health statement was not acceptable in that: [ It was not current (over two years old)]. CCF Handbook, Section 7.2 (Section 19.2, number 1) Record Keeping [SR] The child's (listed on the Supplemental Sheet) last Student Health Exam/Physical form was dated 2/16/21 - over two years old. Technical support provided. 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program.

The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 The staff listed on the Supplemental sheet started working on 12/1/22 and had not started the 40 hour Introductory training. She started it during this inspection. Technical support provided. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel.
