An infant was observed being fed with a propped bottle. CCF Handbook, Section 2.4.4, C (Section 2.1 - Health and Safety, Page 3) General Requirements [SR] Specialist observed that a baby was on infant pillow and was being fed with a propped bottle. Specialist had the teacher finish feeding the bottle to child in her arms. Per Childcare facility handbook on page 13. There must not be any propped bottles. If a child cannot hold the bottle, childcare personnel must hold the bottle during feeding. There must be no automatic feeding devices unless prescribed by a doctor and documentation is available in the childs file. Due Date Completed at time of inspection

The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed staff files and some didnt have updated child abuse reporting form; specialist had the director have the staff sign updated form at this inspection and place in employee file. Per Childcare facility handbook on page 61. Child Abuse & Neglect Reporting Requirements, which must be signed on or before hire date and annually thereafter by all child care personnel. A safety deficiency having a moderate potential for harm to the children in care was observed. CCF Handbook, Section 3.1 Physical Environment [SR] Specialist observed a mop bucket in the child's bathroom with dirty water and a mop in it. The teacher dumped water out of mop bucket. The Child care facility handbook states on page 20. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Due Date Completed at time of inspection

The ground cover or other protective surface under the [slides and under climbing areas] was not maintained. CCF Handbook, Section 3.12, D Sanitation and Equipment [SR] Specialist observed that there was not enough mulch in the fall zones around playground equipment. The Child care facility handbook states on page 39. If the ground cover in place is loose ground cover (such as, but not limited to mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. Due Date 12/3/2025 TRAINING

An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Specialist observed the one-year-old classroom door does not close; you have to lift door to close it. Needs to be repaired. Specialist observed that the door air vent is missing screws to hold it in place. A mildew/sewer smell in one -year old classroom, Director has stated in email that the sewer issue has been fixed. In facility handbook on pg. 20 states General Health and Safety Requirements A. Personnel records or copies of records were not being maintained at the facility and available for review by the licensing authority. CCF Handbook, Section 7.4 Record Keeping [SR] The facility did not have two personnel records available for specialist to review at this inspection. In the child care facility handbook states on pg. 61. 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. D. Copies of training information and credentials as applicable. E. For the driver only: Florida drivers license and driver physical examination documentation. A copy of the drivers license and the physician certification, or another form containing the same elements of the physician certification, granting medical approval to operate the vehicle, and valid certificate(s) of course completion for first aid training, pediatric cardiopulmonary resuscitation (CPR) procedures, and background screening must also be maintained in the drivers personnel file. Child/ren were placed in a crib, playpen, play yard or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. CCF Handbook, Section 3.6.4, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Specialist pointed out to director that a child can't have bib on while sleeping in crib, the bib was removed from the child at time of inspection. In the childcare facility handbook on page26 3.6.4 Safe Sleep C. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards, other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard.

Child Care personnel, who were not in compliance with training requirements when they left the industry, did not complete required training and any new mandated training before returning to the industry. CCF Handbook, Section 4.3, B There are currently 9 employees. All 9 employee records were reviewed. One employee, HT, left the child care industry without completing the Intro training and Early Literacy training. The facility hired HT without the trainings completed. The Child Care Facility Handbook states on page 43 that: 4.3 Break in Service A. In the event an individual leaves the child care industry in compliance with training requirements and returns to the industry either at the same or a different child care facility, he or she will be given 90 days to comply with any new training requirements established during the gap in employment in the child care industry. B. In the event an individual leaves the child care industry not in compliance with the training requirements and returns to the industry either at the same or a different child care facility, he or she must comply with the training requirements described in this section, as well as any new training requirements that may have been added during the gap in employment in the child care industry prior to re- employment.

A ratio of [1] child care personnel for [6] children is required. A ratio of [1] child care personnel for [8] children was observed. s.402.305(4), F.S. (Section 3, numbers 1 9) General Requirements [SR] The one-year-old room had 1 staff person and 8 one-year-old children in attendance. Counselor instructed another staff person to stay in the one-year-old room. Director stated that a new staff person did not show, but she would hire more staff. The Florida Statutes state: (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group.
