      Based on observation the director did not comply with the section cited above as . . . please refer to LIC 809 for a detail description of LPA's observations. . . . which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 06/24/2025

    • Napping Equipment (B) Floor Mats Used for Napping Shall Be: (5) Maintained in a Safe Condition with No Exposed Foam. This Requirement Is Not Met as Evidenced By:CCR 101239.1(b)(5)

      Based on observation the Director did not comply with the section cited above as LPA observed the children's sleeping mat plastic material is tearing off, the cushion is exposed and coming out of the mat which poses a potential health, safety or personal rights risk to persons in care.
