Based on record review at 12:15 p.m., the daily attendance log for children failed to include the time of departure of each child and the first and last name of the person to whom the child was released. The Specialist reviewed the daily attendance records from 8/1/2024 through present, and observed the following: the departure time was missing 3 times; the first and last name of the person to whom the child was released was missing 4 times. Corrective Action: Effective 8/13/2025, S2 will review children's attendance twice daily to ensure compliance with this regulation.
