      Based on record review and interviews at 12 p.m., S5 and S6 failed to document and notify the parent no later than when C1 (1-year-old) was released on the day of the occurrence. On 10/2/2025, between 3:30 p.m. and 4 p.m., S5 and S6 observed rocks in C1's soiled diaper and failed to document and notify O1 of the incident during C1's pick up at 4:36 p.m. Corrective Action: Effective 10/15/2025, S1 stated she will meet with all staff to notify and document on an incident report when rocks are observed in soiled diapers, to ensure compliance with this regulation.

      Based on record review at 11 a.m., S1 failed to provide a written Emergency Medication Plan of Action, updated at least every 6 months for C1, 2-years-old. The authorization on file for Epinephrine is dated 2/2024, and needed to be updated no later than 8/2024. This was corrected during the inspection.

      Based on record review/interview at 1 p.m., the written record for each field trip failed to be maintained for all vehicles while children were being transported. The Specialist observed one field trip record was used to document two different vehicles used to transport separate groups of children and staff on 6/5/2024, 6/19/2024, 6/26/2024, 7/3/2024, 7/10/2024, 7/24/2024, 7/31/2024. The field trip record for 7/3/2024 and 7/31/2024 failed to include the bus driver's name. This was not corrected during the inspection.
