      Based on record review at 2:15 p.m., S1 failed to have documentation that 1 of 4 staff on the premises and accessible to children had a current certification in pediatric first aid through training approved by the Department. S3 failed to have a current certification. A class has not been scheduled. Corrective Action: Effective 5/19/2026, S2 stated she will inform S1 to enroll S3 in a Department approved PFA class, and going forward, will verify all staff files have current certifications to ensure compliance with this regulation.

Corrective Action: Effective 9/3/2025: S1 stated she will obtain a staff file for S7, complete with an application/staff information form, to be maintained at the center for review, to ensure compliance with this regulation.
