Based on record review at 9:00 a.m., S1 failed to ensure the center's staff and owner's daily attendance record accurately reflected persons on the child care premises at any given time as 2 of 5 staff, S1 and S5, were not signed in. Corrective Action: Effective 6/03/2026, S1 stated she will not sign in and failed to provide a corrective action plan for this deficiency.

Based on record review at 10:45 a.m., S1 failed to provide documentation that the entire center and play yard was checked after the last child departed to ensure that no child is left unattended at the center. Upon request, S1 was unable to provide records from March 26, 2026 to May 29, 2026 for the center's end-of-day check. Based on observations during the walk-through, S1 failed to ensure all of the playground equipment was maintained in a clean and safe condition and in good repair. The Specialist observed a Merry-Go-Cycle with 5 out of 5 seats broken and/or cracked. Corrective Action: Effective 6/03/2026, S1 stated she has new seats for the Merry-Go-Cycle and will contact someone to have them installed to ensure compliance with this regulation.

Based on record review/interview at approximately 12:30 p.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time as evidenced by when the daily attendance log for staff was reviewed if only had S2 and S3 signed in. S1 stated that S4 had been at the center but left for lunch but she was not signed in and she also stated that she had not signed into the center. This could not be corrected during the center inspection.

1711: Based on observation at 2:00 p.m., S2 failed to meet the required child to staff ratio for children of the following ages: 8 children age 1- year-old with 1 staff. The required ratio for children of this age is 7 children per 1 staff person. S2 transitioned one of her children to S3's classroom prior the Specialist leaving the center correcting the child to staff ratio.

Based on observation/record review/interviews at approximately 12:35 p.m., children failed to be under supervision at all times. When the Specialist arrived at the center S2 stated she did not know where S1 was and that she was not present in the center. Eight 2-year-old children were asleep on their cots and no adult was in the room with them when the Specialist passed by. When the Specialist returned to the classroom after checking the rooms of S2 and S3, S1 had arrived and was in the room with eight 2-year-old children. Based on observation at approximately 1:00 p.m., items that can be harmful to children, tools, and other potentially dangerous utensils, were not kept in a locked cabinet or other secure place that ensures they are inaccessible to children. Specialist observed that the after school room was being used as a storage. It was housing lawn cutting equipment (blowers and weed eaters), and commercialist extension cords. This could not be corrected during the inspection.

Based on observation at approximately 1:00 p.m., the indoor area failed to be free of hazards as the after school classroom was being used as a storage and had many stacked boxes of classroom supplies, equipment and furniture stacked in a manner that would tip over on the children. This could not be corrected during the inspection.

Based on observation at 12:30 p.m., the center's cribs failed to be free of toys or other soft or loose bedding (including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges) while the child was in the crib as evidence by the Specialist observing C1 in the crib in S3's classroom with a yellow and white teddy bear stuffed animal. S3 removed the bear prior to the Specialist leaving the room.

1909.H. Based on observations at approximately 12:30 p.m. infants (C1, C2, and C3) head/face was covered, S3 was responsible for C1, C2, and C3 and all three infants face/head were covered by their blankets. S3 removed the blankets prior to the Specialist leaving the classroom.
