S11 was first present in the center on 10/27/2025, and should have had seven day training on or by 11/3/2025, and thirty day training on or by 11/27/2025. Based on record review/interview at approximately 1:00 p.m., S1 failed to notify Child Welfare within 24 hours of the following critical incident: On 9/27/2024, S3 (DOH:8/8/2011) informed S1 and S2 that she hit C1, 2-years-old, on the left forearm after having to pick up blocks after C1 pulled them down three to four times. This could not be corrected during the inspection.

      Based on interview at approximately 1:30 p.m., despite the center having a behavior management policy, S3 (DOH:8/8/2011) used a prohibited method of discipline as a child was subject to physical punishment. S3 stated that she was preparing to take her classroom outside and C1, 2-years-old, continually took blocks off the shelf after correcting C1, he hit her and in response she hit him on his left forearm. This could not be corrected during the inspection.
