Based on observations/record review/interview at 9:30 a.m., S1 failed to operate within the licensed capacity of the center. The center's licensed capacity is 41 children, and the Specialist observed 43 children present at the center. S1 stated she did not realize she was over her capacity, and she had unexpected children come in on today. Based on record review and interview at 9 a.m., S1 failed to maintain documentation of a daily attendance record for staff to include their time of arrival. The Specialist observed S2, S3, and S4 present at the center, but they were not signed in on the attendance log. Based on observations at 9:30 a.m., S1 failed to have the outdoor area be free of hazards. The Specialist observed 2 bait boxes that state "contains poison do not touch" accessible to the children on the playground. The Specialist also observed a broken slide with parts on the ground that S1 stated has been broken for weeks and is being repaired. S1 removed the bait boxes from the playground prior to the Specialist's departure. The passenger transportation log failed to have documentation of the time the children got on the vehicle and the time the children were released from the van in the morning on 03/25/2026. Based on observation at 1:45 p.m., S1 failed to have items that can be harmful to children kept in a locked cabinet or other secure place that ensured they are inaccessible to children. The Specialist observed a cabinet underneath the sink in the one and two-year-old classroom that was unlocked with insect spray and a drill that was accessible to the children. S1 removed the insect spray and drill until she can get a proper lock for the cabinet. Based on observation at 1:30 p.m., S1 failed to have the indoor area free of hazards as the Specialist observed adult scissors on S6 and S7's desks in the classrooms and accessible to the children. S1 removed the scissors upon the Specialist's request. Corrective Action: Effective 12/30/2025, S1 stated she will take all scissors and place them in a high cabinet, as well as meet with staff to discuss hazards by 01/09/2025, to ensure compliance with this regulation.

Based on observation at 1:45 p.m., S7 failed to have items that can be harmful to children kept in a locked cabinet or other secure place that ensured they are inaccessible to children. The Specialist observed a cabinet underneath the sink in the one and two-year-old classroom that was unlocked with insect spray and a drill that was accessible to the children. S7 locked the cabinet immediately upon request. Corrective Action: Effective 12/01/2025, S1 stated she will meet with staff by 12/05/2025, to review harmful items being locked up to ensure compliance with this regulation.

Based on observation at 1:45 p.m., S2 and S3 failed to have cribs free of loose blankets while the children were in the crib. The Specialist observed 2 infants in the crib with loose blankets. S2 removed the blankets upon request by the Specialist, prior to the end of the visit. Corrective Action: Effective 12/01/2025, S1 stated she will meet with staff by 12/01/2025, to review cribs being free of loose blankets to ensure compliance with this regulation.

Based on observation at 1:45 p.m., S2 and S3 failed to have written authorization from a physician was available for 1 infant to use a positioning device. The Specialist observed a boppy pillow in a crib with an infant on it during nap time. S2 removed the boppy pillow immediately upon request. Corrective Action: Effective 12/01/2025, S1 stated she will meet with staff to review safe sleep with positioning devices by 12/05/2025, to ensure compliance with this regulation.

Based on observation at 1:45 p.m., S2 and S3 permitted a bib to be worn by two children while asleep. The Specialist observed two infants in cribs asleep with bibs on. S2 removed the bib immediately upon the Specialist's request, prior to the end of the visit. Corrective Action: Effective 12/01/2025, S1 stated she will meet with staff to review safe sleep involving bibs by 12/05/2025, to ensure compliance with this regulation.

Based on observation at 12:30 p.m., S1 failed to have items that can be harmful to children kept in a locked cabinet or other secure place that ensured they are inaccessible to children. The Specialist observed a shovel, rake, and broom on the playground accessible to the children. S1 removed the tools prior to the Specialist's departure. Corrective Action: Effective 09/05/2025, S1 stated she will ensure to check the playground for tools and other items that can be harmful to children daily, to ensure compliance with this regulation.

Based on observation at 12:30 p.m., S3 and S4 failed to have the center's crib free of loose bedding while the child was in the crib. The Specialist observed one infant asleep in a crib with a loose blanket on them. S3 removed the blanket immediately upon request. Corrective Action: Effective 09/05/2025, S1 stated she will review cribs free of toys or loose bedding with the staff by 09/12/2025, to ensure compliance with this regulation.

Based on record review of staff files at 1:30 p.m., S1 failed to have documentation that1 of 9 staff on the premises and accessible to the children have current certification in infant and child CPR through training approved by the Department. S6 failed to have the certification, as her training expired on 06/30/2025. S1 stated she scheduled an infant and child CPR training for 08/19/2025. Corrective Action: Effective 08/04/2025, S1 stated she will make a list of staff's certification and write down reminders for renewals of the trainings to ensure compliance with this regulation.

Based on record review of staff files at 1 p.m., S1 failed to have documentation that 1of 9 staff on the premises and accessible to the children have current certification in pediatric first aid through training approved by the Department. S6 failed to have the certification, as her training expired on 06/30/2025. S1 stated she scheduled a pediatric first aid training for 08/19/2025. Corrective Action: Effective 08/04/2025, S1 stated she will make a list of staff's certification and write down reminders for renewals of the trainings to ensure compliance with this regulation.

Based on observation at 1 p.m., S1 failed to have the indoor area free of hazards as the Specialist observed a hot glue gun plugged in on a low shelf accessible to children. S1 removed the hot glue gun prior to the Specialist's departure. Corrective Action: Effective 08/04/2025, S1 stated she will review hazards with staff by 08/05/2025, to ensure compliance with this regulation.

Based on record review of staff files at 2 p.m., S1 failed to have documentation that 1 of 8 staff on the premises and accessible to the children have current certification in infant and child CPR through training approved by the Department. S5 failed to have the certification, as her training expired on 05/30/2025. S1 stated there is an infant and child CPR training scheduled for 06/09/2025. Corrective Action: Effective 06/03/2025, S1 stated she will make a list of staff's certification and ensure to schedule all trainings timely to ensure compliance with this regulation.

Based on record review of staff files at 2 p.m., S1 failed to have documentation that 1 of 8 staff on the premises and accessible to the children have current certification in pediatric first aid through training approved by the Department. S5 failed to have the certification, as her training expired on 05/30/2025. S1 stated there is a pediatric first aid training scheduled for 06/09/2025. Corrective Action: Effective 06/03/2025, S1 stated she will make a list of staff's certification and ensure to schedule all trainings timely to ensure compliance with this regulation.

Based on record review/interview at 1:45 p.m., S1 failed to document that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The end-of-day check was last documented on 05/12/2025. S1 stated S8 usually completes the end-of-day check when she closes the center, and S1 was unaware it had not been completed. Corrective Action: Effective 06/03/2025, S1 stated she will assign S2 to review the end-of-day check daily to ensure it is documented and completed to ensure compliance with this regulation.

Based on observation at 12:45 p.m., on 05/13/2025, S7 had 6 infants and one-year-olds grouped together during rest time. S1 stated she was unaware the infants could not be grouped during rest time. Ratio was corrected at 1:00 p.m., when S8 came back from her break. Corrective Action: Effective 05/13/2025, S1 stated she will review child to staff ratio with all staff by the end of the day to ensure compliance with this regulation.

Based on observation at 12:45 p.m., on 05/13/2025, children were not under supervision at all times. During the walk-through of the center, the Specialist opened the door to the classroom and found 6 one-year-olds sleeping while S5 was on the ground covered up with a blanket with her eyes closed facing the wall. The Specialist walked over and touched the staff member to wake her. Corrective Action: Effective 05/13/2025, S1 stated she will meet with all staff by the end of the day to review supervision to ensure compliance with this regulation.

Based on record review/interview at 1:30 p.m., on 05/13/2025, S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, May, and June. S1 stated she completed the tornado drills, but is unsure where the documentation is located. Corrective Action: Effective 05/13/2025, S1 stated she will pick a specific place to keep the tornado drill documentation for review to ensure compliance with this regulation.

Based on record review/interview at 10:30 a.m., S7 (DOH 08/19/2024) failed to have current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. S7 stated she is usually alone with the children in the classroom, and her previous CPR and First Aid certification expired on 09/30/2024. S1 scheduled a CPR and First Aid training for 11/05/2024.
