Based on record review/interviews at 12:21 p.m., S1 failed to notify the parent immediately and DCFS within 24 hours of the following critical incident: On 12/31/2024, at approximately 9:55 a.m., O1 called the center and spoke with S1 stating that she witnessed S21 pull C1s, age one-year-old, hair and push his head down on the cot on 12/30/2024 at approximately 2:31 p.m. S1 notified the parent of C1 on 1/3/2025. LDOE was notified on 12/31/2024. Specialist was unable to validate the incident.
