S5’s first day working at the center was 03/24/2025. S5 completed the Online Child Abuse and Neglect Mandated reporter Training on 10/29/2025. S1’s mandated reporter certificate expired on 12/24/2024. S2’s mandated reporter certificate expired on 8/23/2025. Corrective Action: Effective 11/19/2025, S1 stated she will set a reminder 30 days prior to certificate expiration to ensure compliance with this regulation.

Specialists observed a radio cord hanging from bookshelf in classroom # 2. S4 removed the radio cord before the specialists left out of the room. Corrective Action: Effective 11/19/2025, S1 stated she will complete daily walkthroughs of the classrooms ensure compliance with this regulation.

Specialists observed the safety box posted from April to June 2024. Corrective Action: Effective 11/19/2025, S1 stated she will check the daily observation logs at the beginning of each day to ensure compliance with this regulation.
