      Based on record review at 10:21 a.m., S1 failed to have documentation of said determination available for S8 (DOH 10-09-2025), on the center’s CCCBC roster at all times. This was corrected prior to Specialists departure. Corrective Action: Effective 03-19-2026, S1 stated she will set a daily reminder to check CCCBC to ensure staff are on the roster to ensure compliance with this regulation.

      1711.A.B.D.E Based on observations/interviews at 10:56 a.m., S1 failed to ensure the required child to staff ratio was met for one year olds. The Specialist observed S4, alone, supervising 10 children, age 1-year-old. The required ratio for this age group is 7 children per 1 staff person. One additional staff was needed to satisfy ratio. Ratio was corrected at 11:01 a.m. when S3 walked back into the classroom. Corrective Action: Effective 02/05/2026, S2 stated she will remind staff and complete a walk-through in each classroom to ensure compliance with this regulation.

      1811.A.2 Based on record review, at 10:09 a.m., S1 failed to have documentation that a CCCBC-based determination of eligibility (CCCBC) was obtained for S5 (DOH 10/13/2021) at least once during a five-year period. S5’s CCCBC expired on 1/11/2026. S5’s staff attendance record indicated that S5 was present on premises on 01/13/2026. S5 became eligible on 01/14/2026. Corrective Action: Effective 02/05/2026, S2 stated she will send staff to complete their CCCBC 90 days before expiration, to ensure compliance with this regulation.

The Specialist observed S5 supervising four children, ages, 2 -to- 3 year old during the walk through. S5’s CCCBC expired on 1/11/2026. S5 left the premises at 12:10 p.m. Corrective Action: Effective 01/12/2026, S2 stated she will ensure all staff have a CCCBC prior to working at the center to ensure compliance with this regulation.
