Based on record review/observation at 3:10 p.m., S1 failed to have documentation that S3 (DOH 2 /7 /2025), S2 (DOH 12/2/2024) and S5 (DOH 3/10 /2025) have current certification in pediatric first aid and CPR within 90 calendar days and prior to assuming sole responsibility for any children. S3 was observed, alone, with 5 children ages, one to two-years-old. S5 was observed, alone, with 9 children ages, three-years-old. S2 was observed, alone, with 6 three to 4-years-old. This was not corrected as S1 did not have anyone to go in the classroom.

      1919.H. Based on observation at 11:27 a.m. S1 failed to have infants that cannot hold a bottle, to be held while being bottle-fed. The Specialist observed C1, and C2, age seven months old, in bouncers with a bottle propped by a blanket. The Specialist also observed S3 place C3, age eight months old, in a crib on her back and gave C3 a bottle to drink.
