Based on interview at 10:30 a.m., S1 failed to notify the Department and Child Welfare within 24 hours of the following critical incident: On 3/16/2025, S1 became aware of allegations by O3 that C1, 7-months-old, received a black eye while at the center. This could not corrected prior to the Specialist departure. Corrective Action: Effective 5/1/2025, S1 stated now that she is aware of the reporting process, she will report allegations received from outside personnel to ensure compliance with this regulation.
