The Specialists observed the following: ¨ In classroom 1, there was a brown purse on top of the shelf near the restroom that was accessible to children ¨ In the main area of building one, there was a brown purse on top of the shelf along the back wall that was accessible to children.       C: Based on record review/interview at 3:00 p.m., S15 failed to notify the Department and Child Welfare within 24 hours and the parent immediately of the following critical incident: On 3/19/2025, at approximately 12:00 p.m. C1 and C2, both 5-years-old, were left alone in the restroom in the School Age Childrens building for about 60 seconds.
