      1713.A.B. Based on observations at 12:00 p.m., S1 failed to ensure children were under supervision at all times. The Specialist observed 10 children, ages 1 to 2-year olds, unsupervised in Classroom 2 and 8 children, ages 2 to 3-years old, unsupervised in Classroom 1. This was corrected when S2 arrived at the center at 12:19 p.m. to supervise Classroom 1 and Classroom 2 that share a common doorway. Corrective Action: Effective 4/14/2026, S1 stated she will complete daily walkthrough to ensure compliance with this regulation.

      1711.B.2. Based on observation at 12:00 p.m., S1 failed to have a minimum of 2 child care staff present at an early learning center when more than four children are present. The Specialist observed S3 caring for 22 children, ages 5 - months- to 3-year-old, alone, in the center. This was corrected when S2 arrived at the center at 12:19 p.m. Corrective Action: Effective 4/14/2026, S1 stated she will do random walk-throughs throughout the day to ensure compliance with this regulation.

      1711.F. Based on observations at 12:00 p.m., S1 failed to ensure the required staffing were present in the building during rest time to satisfy child/staff ratio. The Specialist observed S3 caring for 22 children, ages 5 - months- to 3-year-old, alone, in the center. Two additional staff were needed to satisfy ratio. Ratio for the 1 to 2 year old classroom was corrected at 12:19 p.m. when S2 arrived on premises and went into the toddler classroom. Ratio for the 2 to 3 year old classroom was corrected at 12:23 p.m. when S1 arrived on premises and went into the 2 year to 3 year old classroom.…

      C.: Based on record review at 12:00 p.m., S1 failed to provide documentation that 2 of 5 staff obtained a minimum of 12 clock hours of continuing education by 8/30/2025,the center’s anniversary year. Corrective Action: Effective 4/14/2026, S1 stated she will set a reminder every month to send her staff a list of upcoming trainings to ensure compliance with this regulation.
