Based on record review at 11:00 a.m., S1 failed to notify DCFS within 24 hours of the of the following critical incident : On 7/29/2025, O1 notified S1 and S4 that S5 stated to her, C1, age 1-year-old, and, C2, age 4-years-old, were being hit and pinched by staff at the center. The Specialists could not validate the allegation. Corrective Action: Effective 8/12/2025, S1 stated she will notify DCFS of all critical incidents within 24 hours to ensure compliance with this regulation.

Based on observation at 10:17 a.m., S1 failed to meet the required child to staff ratio for infants. The required ratio for infants is 5 infants per 1 staff person. The specialists observed S9, alone, with 8 children, ages 7 months to 2 -years-old. S1 corrected child to staff ratio at 10:19 a.m. by placing S10 in the classroom. Corrective Action: Effective 8/12/2025, S1 stated she will ensure when a staff steps out the classroom, another staff will be immediately placed in the classroom to ensure compliance with this regulation.
