      Based on record review/interviews on 03/17/2025, at 9:44 a.m. although a mandated reporter, S20 (DOH 07/07/2014) failed to report abuse of children to the Child Protection Statewide Hotline. S20 observed S19 subjecting C1 (3-years-old), C2 (2-years-old), C3 (3-years-old), C4 (2-years-old) and C5 (3-years-old to the following prohibited methods of discipline: Hitting the children with a fly swatter on their face, arms, and back and poking them with a pointer.       Based on record review/interviews on 03/17/2025, at 10:22 a.m., S19 failed to lift children using proper lifting techniques as on 03/12/2025, S19 was observed in the center's video footage lifting C1, age 3-years-old, by one arm (right arm). S19 lifted C1 off the ground, her feet dangling, and carried her from one side of the classroom to the other side, placing her in time out.
