      Based on record review/interviews on 03/17/2025, at 9:44 a.m. although a mandated reporter, S20 (DOH 07/07/2014) failed to report abuse of children to the Child Protection Statewide Hotline. S20 observed S19 subjecting C1 (3-years-old), C2 (2-years-old), C3 (3-years-old), C4 (2-years-old) and C5 (3-years-old to the following prohibited methods of discipline: Hitting the children with a fly swatter on their face, arms, and back and poking them with a pointer. S19 lifted C1 off the ground, her feet dangling, and carried her from one side of the classroom to the other side, placing her in time out.
