Based on observation at 9:15 a.m., S1 failed to have a minimum of 2 child care staff present at an early learning center when more than four child are present. The Specialist observed S1 supervising 13 children ages 3-years-old to 5-years-old. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/7/2026, S1 the Rayville location is short staffed today and could not send her any help. Based on observation at 9:00 a.m., S1 failed to ensure there is a functional, readily available telephone capable of incoming and outgoing calls at all times at the center. The Specialist attempted to call the center's phone and received the following message: "Your call cannot be completed at this time, please try your call again later." This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/7/2026, S1 stated she will have the main office contact the phone company to service the phone as soon as possible to ensure compliance with this regulation.

Based on observation at 10:00 a.m., S1 failed to ensure the microwave oven is prohibited area accessible to children. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/7/2026, S1 stated she will have the microwaved moved so it is not in the reach of the children to ensure compliance with this regulation.

Based on record review and interview at 10:15 a.m., S1 failed to have additional staff on the bus when transporting any child under four years of age. S1 stated she has been the only staff at the center, so there have been parents volunteering to ride the bus with her. There are children ages 3-years-old to 5-years-old being transported on the bus. Based on record review: S1 failed to have at least two adult to meet child to staff ratio when transporting children. Per the center's field trip log on 3/3/2025, there were 15 children ages 3-years-old to 5-years-old transported with the driver and two parents. There needed to be an additional staff to meet child to staff ratio. Corrective Action: Effective 5/6/2025, S1 stated she will contact her supervisor and inform him that there needs to be additional staff at the center in order to meet child to staff ratio to ensure compliance with this regulation.

Based on record review at 10:30 a.m., S1 failed to maintain a record of all non-vehicular excursion activities done since September 2024 through April 2025. This was not corrected prior to the Specialist departure. Corrective Action: Effective 5/6/2025, S1 stated she will make sure she keeps a log of all the non-vehicular excursion activities going forward to ensure compliance with this regulation.
