      Based on record review at 1:15 p.m., S1 failed to ensure the center's independent contractors daily attendance record included the first and last name of the contractor, the departure time, and the purpose of the visit. This could not be corrected. Corrective Action: Effective 5/7/2026, S1 stated she will double check the log before the contractor leaves the center to make sure it is filled out completely to ensure compliance with this regulation.

      Based on record review at 1:15 p.m., S1 failed to have documentation on file for O1 and O2 that included their name, address, phone number, list of duties performed while at the center. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/7/2026, S1 stated she will have the independent contractors complete the forms when they enter the center to ensure compliance with this regulation.

      Based on observations at 10:00 a.m., S10 and S13 failed to meet the required child to staff ratio. The Specialist observed S10 supervising 16 children, ages 3 years to 4- years-old, alone. S13 was observes supervising 15 children, ages 3-years-old. There needed to be an additional staff in the class with S10 and S13. This was not corrected prior to the Specialist departure. Corrective Action: Effective 5/5/2025, S3 stated she will try to get more staff for the classrooms or make sure substitutes are available to make sure child to staff ratio is met to ensure compliance with this…
