      Based on observations at 12:45 p.m., S1 failed to have a minimum of 2 child care staff present at an early learning center when more than 4 children are present. The Specialist observed S1 supervising 10 children ages 3 - 5-years-old. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/12/2026, S1 stated she will speak to the supervisor at the main center and inform them that additional staff is needed at the center to ensure compliance with this regulation.

      Based on observation and interview at 1:30 p.m., S1 failed to have a minimum of 2 child care staff present at an early learning center when more than four child are present. S1 was supervising 12 children ages 3 - 5-years-old, alone. S1 was left supervising the children alone starting at 9:30 a.m., when S2 signed out and left the premises. This was not corrected prior to the Specialist’s departure. Corrective Action: Effective 2/25/2026, S1 stated S2 was present but had to leave due to her child being ill.       C.: Based on observations at 1:30 p.m., S1 failed to have at least one staff member present trained in medication administration whether the early learning center administers medication or not. This was not corrected prior to the Specialist’s departure. Corrective Action: Effective 2/25/2026, S1 stated she will take the medication training within the next two weeks and she will speak with administration at the main office about having trained staff at the center to ensure compliance with this regulation.
