Based on record review at 9:30 a.m., S1 failed to have documentation on file for O1 that included the person's name, address, phone number, and list of duties performed while at the center. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 2/25/2026, S1 stated she will have all independent contractors complete the form when they visit the center to ensure compliance with this regulation.

1727.A.&B.: Based on record review at 9:15 a.m., S1 failed to have documentation that S7 completed the online child abuse and neglect Mandated Reporter Training provided by DCFS annually. S7's training expired on 2/24/2026. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 2/25/2026, S1 stated S7 is in the process of taking the training, but has not completed it. She will create a spreadsheet to track the staff's trainings to ensure compliance with this regulation.

Based on observation/interview at 9:00 a.m., S1 failed to notify the Department prior to making changes that may have an effect on the license, such as structural changes. While at the center, S1 informed the Specialist that only half of the center is presently being used, due to the other half being renovated. S1 stated the Department has not been notified of the renovations. The floors are being done and classrooms are being painted, and any thing else that needs to be done to the building. S4 failed to have the current certification. This was not corrected prior to the Specialist departure.
