Based on observations, at approximately 12:20 p.m., on 1/16/2026, S11 failed to ensure pacifiers were not attached to childrenâs bibs or clothing as C3, 7-months-old, and C4, 7-months-old, were observed with their pacifiers clipped to their bibs. S11 removed the pacifier clips prior to the Specialist leaving the classroom. Corrective Action: Effective 1/21/2026, S1 stated she will complete a training with all staff on the regulation and the importance and ensuring that pacifiers are not attached to children by 2/6/2026, to ensure compliance with this regulation.
