Based on record review and interviews on 11/17/2025, at 2:15 p.m., S13 failed to ensure children were supervised at all times. On 11/14/2025, C1, 2- years-old, was left unsupervised on the playground from 10:58 a.m. until 11:03 a.m. (5 minutes). When bringing the children back inside the building from playing on the playground, S13 failed to complete a head count to ensure all the children were accounted for. On 11/14/2025, at approximately 10:58 a.m., S13 lifted C2, 2-years-old, by both arms while assisting her from the playground to the inside of the center when outside playtime was over. C.: Based on record review and interviews on 5/6/2025 at 10:00 a.m., S1 failed to notify the Department within 24 hours of being made aware of the following critical incident she observed on the center's video footage on 4/29/2025:- C1, 6-years-old, was repeatedly kicked, hit, and chased by C2, 5-years-old, while in the Sanctuary and while both S2, Classroom Teacher, and S6, Classroom Teacher were present.S2 was placed on a 90-day probation from supervising the school-aged children and from driving the van for afternoon pickup. Based on record review and interview at 10:00 a.m., although the center has a behavior management policy in place, C1, 6 years-old, was observed on the center's camera footage being hit and kicked by C2, 5-years old and being taunted by C3, 5-years-old. Based on record review and interviews at 10:00 a.m., S2, while supervising a group of children, failed to devote her time to supervision of the children; S2 was observed on the center's camera footage completing administrative work. S2 was placed on a 90-day probation from supervising the after-school aged children and from her transportation duties. Corrective Action: Effective 5/6/2025, S1 stated she will continue to walk around and monitor that all staff are supervising children at all times to ensure compliance with this regulation.

Based on interviews and observation of video footage, S15 failed to lift a child using proper lifting techniques. On 04/08/2025 at approximately 8:50 a.m., S15 lifted C1, 3-years-old, by her left arm while assisting her to a classroom table to sit down. S15 was placed on a 3-day suspension from working at the center and a 90 day probation thereafter. All staff received a re-training on the proper lifting techniques following the Bulletin 137 regulation. Based on record review, observation and interviews at 11:00 a.m., S10 failed to ensure children were supervised at all times. On 12/31/2024, C1, 2- years-old, was left unsupervised on the centers playground from 3:18 p.m. until 3:19 p.m. S10 exited the playground with the class at 3:18 p.m., but C1 was not with them, as he was inside of a green and brown toy tree on the playground. He was retrieved unharmed at 3:19 p.m. by S2. C1 was left unsupervised for approximately 1 minute. S10 was placed on probation for a period of 3 months along with a 2 day suspension.

Based on observation/interview at 12:00 p.m., S16 (DOH 5/12/2022) and S17 (DOH 9/16/2024), failed to actively participate with children at all times while in the toddler classroom. Per S1's verbal statement and video observation, on 11/26/2024, at 9:04 a.m., C1, 1-years-old, tripped and fell while running in the classroom. C1s chin area hit the floor and his tooth was chipped. C1 laid crying on the floor for about one minute and 4 seconds. S16 and S17 did not check on him. S1 terminated S16 and S17 on 11/27/2024.
