On 04/08/2025 at approximately 8:50 a.m., S15 lifted C1, 3-years-old, by her left arm while assisting her to a classroom table to sit down. S15 was placed on a 3-day suspension from working at the center and a 90 day probation thereafter.       Based on record review, observation and interviews at 11:00 a.m., S10 failed to ensure children were supervised at all times. On 12/31/2024, C1, 2- years-old, was left unsupervised on the centers playground from 3:18 p.m. until 3:19 p.m. S10 exited the playground with the class at 3:18 p.m., but C1 was not with them, as he was inside of a green and brown toy tree on the playground. He was retrieved unharmed at 3:19 p.m. by S2. C1 was left unsupervised for approximately 1 minute. S10 was placed on probation for a period of 3 months along with a 2 day suspension.

      Based on observation/interview at 12:00 p.m., S16 (DOH 5/12/2022) and S17 (DOH 9/16/2024), failed to actively participate with children at all times while in the toddler classroom. Per S1's verbal statement and video observation, on 11/26/2024, at 9:04 a.m., C1, 1-years-old, tripped and fell while running in the classroom. C1s chin area hit the floor and his tooth was chipped. C1 laid crying on the floor for about one minute and 4 seconds. S16 and S17 did not check on him. S1 terminated S16 and S17 on 11/27/2024.
