Based on observations at 1:30 p.m., S3 failed to ensure that bibs are not worn by any child while sleeping. The Specialist observed 2 infants ages 4 months and 6 months, asleep while wearing a bib. Based on record review and interviews on 07/17/2025, at 3:45 p.m., although the center has developed and implemented a Behavior Management Policy, S2 (DOH 12/09/2024), used a prohibited method of discipline.
