Based on record review and interviews on 02/27/2026, at 12 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for S8, prior to the person being present at the center or performing services. S8's first day working at the center was on 01/12/2026, and she received an eligible CCCBC on 02/10/2026. S1 stated she thought S8 could be a provisional staff member until she received an eligible CCCBC. Based on record review/interview at 8:15 am, S1 failed to document the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The last visual check of the center was conducted on 6/17/2025. Corrective Action: Effective 7/01/2025, S1 will place the end-of-day check at the front door as a reminder to complete the check daily, and she and S9 will both sign the check upon exiting the center, to ensure compliance with this regulation.

Based on observation at 8:45 a.m., the Outdoor play space failed to have a direct exit from the center that ensured children are continuously protected by a permanent fence or other permanent barrier while going to and from the center to the outdoor play space. Corrective Action: Effective 7/01/2025, S1 will contact the Department and appropriate agencies by 7/02/2025 to begin the process of adding an additional fence between the center and the play space. CPR and First Aid Certification: Based on record review at 9:45 a.m., S1, and S10 failed to have documentation that 3 of 10 staff on the premises and accessible to children have current certification in pediatric first aid and CPR before assuming sole responsibility for any children. S6 (DOH 9/23/2024), S7 (DOH 10/14/2024), and S8 (DOH 8/15/2024) failed to have the current certification. The Specialist observed S6, S7, and S8 supervising children, alone in a classroom during the visit.
