The Specialist observed an exposed cable/electrical cord on the playground, accessible to children. 1103.A Based on interview at 11:45 a.m., S1 failed to notify the Department within 24 hours of the following critical incident: On 02/11/2026, S1 was notified of an allegation by the Office of Public Health, that a complaint was made of the smell mold when you enter the center, there are sick children at the center that are not being sent home, the sleep mats are torn, the center does not have hot water, there is a hole in the floor under the rug in the hall bathroom, and the baseboards in the bathroom are falling off and could pose a safety hazard to the children. Based on record review, at 11:30 a.m., the center's daily attendance records for visitors failed to accurately reflect when a representative from the Office of Public Health, was on the premises. S1 stated on 02/11/2026, a Representative from the Office of Public Health, visited the center to investigate allegations of items that are harmful to children and failed to sign the visitor’s log. Corrective Action: Effective 03/18/2026, S1 stated she will check the visitor's log after each visitor leaves to ensure compliance with this regulation.

1515.A. 1.: Based on record review at 12:30 a.m., S1 failed to have a cumulative file maintained at the center. The Specialist observed there was no record for C10, 9-months-old, on file on the premises to include the child's birthdate, sex, date of admission, name of parents, home address, phone numbers where parents can be reached while in care, name of person to contact in emergency if parents cannot be located promptly, special concerns, or dietary needs, restrictions or food allergies or intolerances if applicable. S2’s last Mandated Reporter training was completed on 08/24/2024, and S4's Mandated Reporter training was completed on 06/6/2024. Corrective Action: Effective 03/18/2026, S1 stated she will set a 30-day calendar reminder prior to the expiration to ensure compliance with this regulation.

Based on observation at 11:45 a.m., S1 failed to keep the outdoor areas hazard-free. The Specialist observed an exposed chord and an A.C. unit with a broken lattice fence. The Specialist observed a broken baseboard in the children’s bathroom underneath the sink, accessible to the children. Corrective Action: Effective 3/18/2026, S1 stated at the end of the day she will walk the playground to ensure the fence is up and secure to ensure compliance with this regulation.

Based on observation and interview 12:30 p.m., S1 failed to have an appropriate sleeping arrangement available for each child aged one and older. The Specialist observed 3 mats in the three-year-old room with torn vinyl exposing the foam underneath. The Specialist also observed the children sleeping on the mats during the visit. Corrective Action: Effective 03/18/2026, S1 stated she will have staff immediately report to her whenever mats are torn to ensure compliance with this regulation.

Based on record review at 12 p.m., S1 failed to document the physical condition of each child for signs of illness or infection and include an explanation from the parent. The Specialist observed daily observation reports, from 01/5/2026 to 2/18/2026, documenting nothing observed, although S1 stated children sometimes come into the center with a runny nose on 02/06/2026. Corrective Action: Effective 03/18/2026, S1 stated she will document all signs of illness on the observation report to ensure compliance with this regulation.

Based on record review and interview at 10:30 a.m., S1 failed to include the first and last name of the person or entity to whom the child is released. The Specialist reviewed the Daily Attendance Log for August 18-19 2025. S1 failed to complete the Children's Daily Attendance record, and the record did not include the first and last names of the children or the time of departure for all children. Based on the record review at 11:00 a.m. S1 did not maintain an accurate documentation of the daily attendance record for visitors to include the name, date of arrival and departure times, and the purpose of the visit. The Specialist observed that there was no evidence of a visitor log being maintained. Corrective Action: Effective 06/10/ 2025, S1 stated that she will create a visitor's log to ensure compliance with this regulation.

Based on observations at 10:15 a.m. S3 failed to follow the center's electronic device policy. Electronic device activities, including but not limited to television, are prohibited for children under the age of 2. The Specialist observed three 1-year-old children watching television from 10:30 a.m. until 11:00 a.m. Corrective Action: Effective 06/10/2025, S1 will no longer allow 1-year-olds to watch television, to ensure compliance with this regulation.
