According to center records, S5 (DOH: 3/02/2026), worked from 3/02/2026, until present, without a CCCBC-based determination of eligibility. S1 instructed S5 to leave center premises immediately after scheduling appointment for fingerprints.       Based on record review and interviews at 12:45 p.m., S1 failed to immediately notify O1 of the following critical incident: On 6/05/2025, at 9:43 a.m., S10 fell with over a bouncer in the infant room with C1 (3 months old), in her arms. O1 was at 10:22 a.m. A critical incident report was submitted to the Department and Child Welfare were notified on 6/05/2025. Corrective Action: Effective 6/23/2025, S1 stated she will immediately notify parent of an critical incident without gathering additional information to ensure compliance with this regulation.
