The Specialist observed a pacifier attached, by a clip, to the clothing of C1, age nine-months-old. S2 removed the clip during the licensing visit. Corrective Action: Effective 09/08/2025, S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.

The Specialists observed several broken and missing boards in the fencing of the outdoor play area that leads to the parking lot not affiliated with the center.
