Based on record review at 11:00 a.m., S1 failed to have documentation that S9 completed the LDE Key Training Module 1 within seven days of the first day present at the center, or the LDE Key Training Module 2 and 3 within 30 days of the first day present at the center. S9âs first day working at the center was 09/2/2025. Corrective Action: Effective 11/06/2025, S1 stated she will have newly hired staff complete required training within the first 5 days of hire to ensure compliance with this regulation.
