Based on record review/interview on 12/1/2025 at 1:00 p.m., S3 (DOH:8/10/2021) failed to use proper lifting techniques as she lifted C1, 3-year-old, by the arms while moving him away from a group of children to another area of the room. Corrective Action: Effective 12/1/2025, S1 will retrain all staff on how to lift children while de-escalating a situation to ensure compliance with this regulation.

Corrective Action: Effective 5/19/2025, S1 will report all allegations against the center and staff to the Department and Child Welfare to ensure compliance with this regulation.
