General Requirements [SR] One teacher was left in a room with 12 3-year-olds and 8 4-year-oilds, while the second teacher left the room to answer the door. After answering the door, the teacher returned to the classroom. The Child Care Facility Handbook states on page 12: 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times.       The facility did not have an adequate number of cribs, portable cribs or playpens with sides for the number of children up to one- year old in care. CCF Handbook, Section 3.6.3 Physical Environment [SR] Two infants were laying on a quilted blanket on the floor. One child was asleep on its stomach, and the other child was awake on its back. A third infants was in a bouncy chair. While there were an adequate number of cribs, the teacher decided to allow the sleeping child to sleep on the quilted blanket on the floor. The teacher moved the child to a crib during this inspection. The Child Care Facility Handbook states on page 25: 3.6.3 Crib Requirements Children up to one year of age must be in a crib or playpen/play yard with sides for napping. Crib sides must be secured while there is an infant in the crib. Bar spacing must not exceed two and three-eighths inches. Cribs or playpen/play yards must meet the regulations as outlined in Title 16, Parts 1219, 1220 & 1221 Code of Federal Regulations, 2014, which is incorporated by reference in 65C- 22.001(7)(w), F.A.C. In the event an individual leaves the child care industry not in compliance with the training requirements and returns to the industry either at the same or a different child care facility, he or she must comply with the training requirements described in this section, as well as any new training requirements that may have been added during the gap in employment in the child care industry prior to re- employment.       Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B General Requirements [SR] A staff person reported that one of the teachers was not following the Discipline Policy established by the facility and provided to the parents. The Designated Representative will verbally review the policy with the teacher and have her sign the policy to place in her staff file. The Child Care Facility Handbook states on page 19: 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies.       A minimum distance of 18'' was not maintained between each napping/sleeping space. The Child Care Facility Handbook states on page 25: C. A minimum of 18 inches must be maintained around individual napping and sleeping spaces. A maximum of two sides of a napping or sleeping space may be against a solid barrier, such as the wall. The solid side of a crib does not meet the requirements for a solid barrier.       Soiled or wet diapers, clothing, or linens were not changed immediately. CCF Handbook, Section 3.10.2, Sanitation and Equipment [SR] A child sustained a rash due to a pull up being left on without changing when soiled. The child no longer attends the facility. The facility is now using a sharpie and marking the actual diaper or pull up with the time it was changed at each diaper change. The Child Care Facility Handbook states on page 37: 3.10.2 Diapering Requirements H. There must be a supply of clean diapers, clothing, and linens at all times. I. When diapers, clothing or linens that are being used become soiled or wet, they are to be changed immediately and properly disposed. J. Soiled or wet disposable diapers must be disposed of in a plastic lined, securely covered container that is not accessible to the children. The container must be emptied, cleaned and sanitized or disinfected, at least, daily. K. Soiled cloth diapers must be emptied of feces in the toilet and soiled or wet cloth diapers shall be placed in a securely covered container that is not accessible to the children. The container must be emptied, cleaned and sanitized or disinfected, at least, daily. L.       Supervision of children in the [3 years and up] group was inadequate in that [2 children were left in the bathroom unsupervised.]. CCF Handbook, Section 2.4 (Section 2.1 - Health and Safety, Page 3) General Requirements [SR] Specialist observed 2 three year old children alone in the restroom unsupervised. The restroom is in the hallway. The director stood in the doorway of her classroom so that she could see the children in the restroom until they returned to their class. Technical assistance was given regarding the need for an extra staff member that can accompany children into the restrooms since they are not inside the classrooms and easily supervised. The Child Care Facility Handbook states on page 13: H.       The play equipment was not maintained in a safe or sanitary condition for the children to use in that [paint is chipping on 3 peices of equipment.]. CCF Handbook, Section 3.12, B and F (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Specialist observed equipment on the playground that had chipping paint on 3 pieces of equipment. Xylophone - The frame paint is chipping. School bus frame - paint is chipping. Bike merry-go-round - paint is chipping. Equipment needs to be repainted by September 8, 2025. The Child Care Facility Handbook states on page 38: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. CCF Handbook, Section 7.1, B (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed all 34 children files. 3 children files were out of date. One child was not present, the parents were all called and the children will not return until the immunization forms are up to date. The Child Care Facility Handbook states on page 59: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.       A complete CF-FSP Form 5131, Background Screening and Personnel File Requirements, was not on file for all employees. Three staff members did not have the 5131 Forms completed and in their files. Director completed forms while specialist was still present. The Child Care Facility Handbook states on page 62: A.       Harmful items including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials were accessible to children in care. CCF Handbook, Section 3.2, B Physical Environment [SR] Specialist observed a spray bottle with bleach int it left on a desk in reach of children. When pointed out the teacher removed the spray bottle stored it in a cabinet out of reach of the children.

    • Fire Drills & Emergency Preparedness CCF Handbook, Section 3.823-06

      During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care [since February 27, 2025.]. CCF Handbook, Section 3.8.4, A Physical Environment [SR] The Specialist reviewed the fire drill log. The last fire drill was on February 27, 2025. 7 staff and 51 children participated. The Specialist was not able to conduct a fire drill at the time of inspection. The Specialist has given the provider until April 30, 2025, to conduct a fire drill. The provider will advise the specialist when the fire drill has been conducted.

    • Diapering CCF Handbook, Section 3.1030-07

      Items unrelated to diaper changing were stored in the diaper changing area or placed on the diaper changing table/surface. CCF Handbook, Section 3.10.2, G Sanitation and Equipment [SR] Specialist observed the attendance log on the diapering surface. Specialist pointed out the attendance log to the Designated Representative who removed the attendance log for the diapering surface.

      Children did not receive supervision as required by their age or required needs while toileting or bathing. CCF Handbook, Section 2.4.1, H General Requirements [SR] Specialist and Early Learning Coalition representative observed a child left in a restroom without supervision. Specialist waited until the child was finished and returned to the classroom. Teacher was made aware that the child needs to be accompanied to the bathroom. The Child Care Facility handbook states on page 13: H. Children must receive supervision and care in accordance with their age and required needs and be accounted for at all times while bathing or toileting.

    • Credentialed Staff CCF Handbook, Section 4.6 & 4.734-04

      The director responsible for the daily operation of the program did not have an active Director Credential. CCF Handbook, Section 4.7 Specialist had been notified timely by the facility that they had lost their director. They are currently on a provisional license. The Child Care Facility Handbook states on page 47: A. Each child care facility must have a credentialed director who is on-site a majority of hours, excluding weekends and evening hours that the facility is in operation. Documentation of majority of hours must be maintained and available for review by the licensing authority.

      The diapering area was not physically separated from the food preparation, service or feeding area. CCF Handbook, Section 3.10.2, D Sanitation and Equipment [SR] Specialist observed that while the diaper changing pad itself had been removed from the table the diaper changing table service was being used to rest the infant solid foods on and teacher was feeding the children from there. Food was removed at the time of inspection. The Child Care Facility Handbook states on page 37: D. The diaper changing area must be physically separated from the food preparation, food service, and feeding area.

    • Diapering CCF Handbook, Section 3.1030-06

      Diaper changing occurred on a surface that was not impermeable. CCF Handbook, Section 3.10.2, E Sanitation and Equipment [SR] Specialist observed a cloth changing mat cover in the one-year-old room. It was removed while specialist was in the room. The Child Care Facility Handbook states on page 37: E. When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use.

    • Outdoor Equipment CCF Handbook, Section 3.1232-07

      The ground cover or other protective surface under the [two slides and rocking bus] was not maintained. CCF Handbook, Section 3.12, D Sanitation and Equipment [SR] Specialist observed that there was insufficient mulch under the two slide structures and the rocking bus on the playground. The Child Care Facility Handbook states on page 39: 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area.

    • Training Requirements CCF Handbook, Section 433-03

      The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Specialist observed that one teacher had not started the introductory training within 90 days of beginning in the industry. The Child Care Facility Handbook states on page 39: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel.

    • Training Requirements CCF Handbook, Section 433-09

      The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 Training [SR] Specialist observed that one staff member had not completed safe sleep within 30 days of hire. The Child Care Facility Handbook states on page 41: 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Department's training transcript in the child care personnel record.

    • Student Health and Records CCF Handbook, Section 7.242-01

      Child(ren) did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment. CCF Handbook, Section 7.2, C Record Keeping [SR] Specialist observed that one child who had enrolled on 10/28/2024 did not have a Student Health Exam on file. The Child Care Facility Handbook states on page 60: Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program.

CCF Handbook, Section 3.2, B Physical Environment [SR] 2 classroom drawers that were not locked contained lysol spray and diaper cream. These items were removed at the time of this inspection. The CCF Handbook states that hazardous materials must not be accessible to the children in care. The items were removed during this inspection.

      Supervision of children in the [3] group was inadequate in that [a child fell off of a changing table.]. CCF Handbook, Section 2.4 General Requirements [SR] A 3 year old child at this facility, who was not potty trained, was taken to the infant room to be changed. The child was put on the changing table used for the younger children and fell off of the table, breaking her collar bone. This high changing table should not be used for older children and the changing mat should fit securely on the table with the child securely strapped. The CCF Handbook states that child care personnel must position themselves so that all children can be directly supervised.

The program director attempted to get a signature on an incident report on 5/1/2023. The parent refused to sign the report. The CCF Handbook states that documentation must be shared with the custodial parent or legal guardian on the date of occurrence.
