Based on record review at 10:30 a.m., S1 failed to have documentation that 4 of 10 staff on the premises and accessible to children had a current certification in pediatric first aid through training approved by the Department. S1, S5, S6, and S10 failed to have the current certification. A class has not been scheduled. Corrective Action: Effective 11/21/2025, S1 stated she will contact the CPR trainer in an attempt to take the PFA course addition, or schedule a new Department-approved PFA class, to ensure compliance with this regulation.

C.: Based on record review at 11:30 a.m., S1 lacked documentation that S5, S6, and S11 completed orientation requirements within seven days of the first day present at the center and prior to having sole responsibility for any children. S5, S6, and S11 failed to complete and have documentation for verification of the LDE Key Training Modules 1-3; S5 failed to have documentation of the DCFS Mandated Reporter Training. Based on record review at 12 p.m., S1 failed to have documentation of a current certification in CPR and pediatric first aid for S5 (DOH: 5/20/2025), S6 (DOH: 5/26/2025), and S11 (DOH: 8/25/2025) within 90 days from their date of hire and prior to assuming sole responsibility for any children. Corrective Action: Effective 9/11/2025, S3 stated she will enroll all staff lacking a CPF/PFA certification in a Department-approved class ASAP and going forward will remind new staff that they are not to be alone with children prior to obtaining their CPR/PFA certification, to ensure compliance…

Based on record review at 1 p.m., S1 failed to have S7's CCCBC-based determination of eligibility from the Department available for review on the center's CCCBC roster. S7's date of hire and first day working was 8/4/2025. Corrective Action: Effective 9/11/2025, S3 stated she will add S7 back to the center's CCCBC roster and verify all staff are maintained on the roster for eligibility verification to ensure compliance with this regulation.

Based on observation at 9:30 a.m., child to staff ratio failed to be met for the following group of children: 6 infants, with 1 staff. The required ratio for children of this age is 5 children per 1 staff. This failed to be corrected during the inspection. Corrective Action: Effective 4/22/2025, S1 stated she will retrain staff on child/staff ratio guidelines and instruct them to notify her when their classroom is at max or exceeding ratio limits so adjustments can be made, to ensure compliance with this regulation.

Based on record review at 10 a.m., S1 failed to have documentation that 1 of 11 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S13 failed to have current certification. A class has not yet been scheduled. Corrective Action: Effective 4/22/2025, S1 stated she will schedule a class ASAP for the staff lacking a current CPR certification and be more diligent in keeping up with staff's CPR expiration dates, to ensure compliance with this regulation.

Based on record review at 10 a.m., S1 failed to have documentation that 1 of 11 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S13 failed to have current certification. A class has not yet been scheduled. Corrective Action: Effective 4/22/2025, S1 stated she will schedule a class ASAP for the staff lacking a current PFA certification and be more diligent in keeping up with staff's PFA expiration dates, to ensure compliance with this regulation.

Based on record review/observation 12 p.m., a CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for each independent contractor, O1, O2, and O3, prior to them being present at the center or performing services. O1, O2, and O3's based determination of eligibility failed to be available for review on the centers CCCBC roster, their eligibility status failed to be verified on the date they were present at the center, and per center records, they were not accompanied by center staff not being counted in child to staff ratio.…

Based on record review at 12:30 p.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time as S5 was present and not signed in on the log; S7 failed to sign out for break and then back in when she returned to the center; S1 failed to sign in when she arrived to the center; and S8 failed to sign out when she left the center. This was corrected during the inspection.

Based on observations at 12:30 p.m., children failed to be under supervision at all times as the Specialist observed S6 in a classroom asleep with ten 2-year-olds during naptime. This was corrected during the inspection.

Based on record review/observations at 12:15 a.m., S4 (DOH: 12/26/2024) failed to have current certification in pediatric first aid and CPR prior to assuming sole responsibility for ten 3-to-4-year-olds.
